Filament "Swirling" when coming from the extruder???

Everything on the printer is in the right place, but when i tested it out the filament started spiraling as it came out...
I am beyond confused by this. If someone could help me it would make my day. :)

Post photo.

- coriolis force plus wind
- overheated carbonized plastic piece in hotend
- bad shape of output nozzle (probably after crash to bed)

TIP: tight all screws and wheels (except right side of Z axis) and re-align frame while assembly. And silence it.

So that we can solve the problem better.
